The Department of Social Welfare invites to join its team a colleague who will perform the functions of a case manager in working with persons with mental and/or intellectual disabilities.

Work under a fixed-term project work contract for 5 years.

Salary - official salary coefficient 1.34.

By joining the team you:

  • You will assess a person's need for social and other comprehensive services;
  • inform and consult about the possibility of receiving social, healthcare, employment, legal and other complex services
  • you will organize case management meetings;
  • you will mediate when a person applies to other institutions;
  • if necessary, you will accompany the person to complex assistance providers;
  • you will systematize quantitative and qualitative information about the services provided.

We hope you:

  • You have at least a higher university education in social sciences with a bachelor's degree or an equivalent education or a higher college education with a professional bachelor's degree or an equivalent education;
  • you have at least three years of experience working with vulnerable groups in society and/or at least one year of experience working with persons with mental and/or intellectual disabilities or practical work experience with mental disabilities acquired during studies of social work, psychology, special pedagogy and/or persons with intellectual disabilities;
  • you know how to work with a computer using the MS Office software package and know the requirements of the rules for preparing documents.
  • you are familiar with the application of the case management model, providing individualized services and coordinated support for persons with mental and/or intellectual disabilities
