Nature of work
Assess the social needs of patients and their relatives, provide social consultations.
Plan and organize the provision of social assistance to patients during the rehabilitation and treatment period.
Inform patients about their social guarantees, social services and the possibilities of obtaining them.
Cooperate with doctors, nurses, physiotherapists, occupational therapists and other specialists in order to ensure comprehensive assistance to patients.
Help patients and their relatives solve social, psychological and adaptation problems related to their health condition or disability.
Prepare, manage and complete social work documentation, submit reports according to the established procedure.
Participate in the activities of a multidisciplinary team, in discussions of patient cases and in quality improvement processes.
Ensure compliance with the principles of confidentiality, professional ethics and patients' rights.
If necessary, participate in educational and preventive activities for patients and their relatives.

Employee requirements
Higher university or college education in social work and social work qualification.
At least 1 year of work experience in the field of social work (experience in a healthcare, rehabilitation or social services institution is an advantage).
Knowledge of the legal acts regulating social services and healthcare of the Republic of Lithuania.
Ability to assess the social needs of patients, draw up assistance plans and coordinate the provision of social services.
Good communication and cooperation skills, ability to work in a team with doctors, nurses, physiotherapists and other specialists.
Ability to work independently, plan and organize one's work, make decisions within the limits of competence.
Ability to manage documentation, prepare reports, work with a computer (MS Office programs).
Empathy, responsibility, initiative, orientation to patients' needs.
Fluent in spoken and written Lithuanian; knowledge of foreign languages would be an advantage.
Impeccable reputation and ability to adhere to the principles of professional ethics.
At least 1 year of work experience in the field of social work (experience in a healthcare, rehabilitation or social services institution is an advantage).
